Use this form to set up transfer on death for cars and other motor vehicles in Ohio. This will let you transfer your car to your loved ones without a will and avoid probate court.
If you're married, your spouse can transfer an unlimited number of vehicles, worth up to $65,000, to themselves after you die using the BMV Surviving Spouse Affidavit without this TOD.

- Take the form to a notary, sign it and have it stamped.
- Make a copy for yourself.
- Submit the form at your county's Clerk of Courts Title Office.When you submit the form, they will reissue a title that includes your TOD. There will be a small fee for the new title.
